• Before you begin please make sure you have your hospital number to hand. This can be found on your appointment letter.
• Please note it will not be possible to cancel / rebook your appointment online if it is within 1 working day of the appointment date. Please contact 01722 345543 in this circumstance.
• Please be aware that if you select ‘cancel’ appointment no further appointments will be arranged and you will be discharged back to your referrer. A letter will be sent to inform them of this. If you wish to remain under the care of Salisbury NHS Foundation Trust then please ensure you select ‘rebook’ appointment and a further appointment will be arranged and sent to you in due course.
